The Risk Coordinator is solely responsible for submitting insurance claims on behalf of MGA with the exception of automobile accident & workers compensation claims which will be explained later in this document. This coordinator position handles filling out all required insurance forms, gathering pictures, witness statements and submitting them to the proper agencies for notification and processing.
The state driver that is involved in the motor vehicle accident whether driving a state vehicle or a personally owned vehicle (POV) is solely responsible for reporting the accident and meeting all other criteria necessary for starting the claim process. If you are injured or are a witness to another employee being injured while on the job. The Office of Human Resources (OHR) is responsible for reporting all workers compensation claims to the state. If the injury is of a serious nature call 911 and get help immediately en route. If the injury is not life threatening contact OHR to report the accident and also to find out which preferred doctor they need you to go to. There are forms located on the risk management website that will need to be filled out in order to get the facts and document the incident while things are still fresh on your mind. The documents used for injuries/incidents are-Accident Witness Statement, Employee Report of Injury, Incident Report and Supervisor’s Accident Investigation form. Each form has instructions at the top for who needs to fill them out and once completed should be forwarded to OHR here: and copied to for file.
